lib/classifier.js:456signatureOf truncates long string arguments (command/content) to the first 80 characters plus a length marker; index.js (lines 50-56) grants next() without asking once a signature reaches autoAllowThreshold. Two different RISKY-level commands with the same 80-character prefix and the same total length therefore share a signature: after the user approves such a command 3 times (default), the differing command is silently auto-allowed. Scope is limited to RISKY (redlines still always ask by default) and requires a prior approval of a near-identical command.
Static scan's two criticals are false positives: the flagged rm/ssh/scp regexes are this guard's own blocklist; the plugin never executes commands or touches the network. Two low detection gaps.
Rule (2026-10-24): when a dynamic conclusion exists it is authoritative (it may raise or lower the level); without one, the static/vet fused tier applies. The audit never writes the exemption list (exemptions are manual only); a wrong conclusion is withdrawn by human review and the tier falls back to static/vet.
