index.js:88Every tool call spawns PowerShell that evaluates a local script with the host's full inherited environment (line 81: `...process.env`) and user privileges. The Node layer only transports JSON on stdin and enforces the confirm flag; the actual COM calls, the attachment write path for outlook_save_attachment (attacker-influenced attachment name plus caller-supplied target directory) and the OLK_CONFIRM check live in the unreviewed .ps1 files, so argument handling and path sanitisation on the privileged side could not be verified.
Outlook COM bridge is coherent and mostly defensive, but the advertised human approval gate is entirely agent-controlled and the privileged PowerShell payloads were not in the reviewed set.
Rule (2026-10-24): when a dynamic conclusion exists it is authoritative (it may raise or lower the level); without one, the static/vet fused tier applies. The audit never writes the exemption list (exemptions are manual only); a wrong conclusion is withdrawn by human review and the tier falls back to static/vet.
