engine.js:812`name` is only checked against the in-memory index by uniqueName (which returns it unchanged when no document of that name exists). A name such as `..\\..\\Startup\\payload.bat` produces `files\\..\\..\\Startup\\payload.bat`, so POST /knowledge/ingest (impl.js route -> handle({op:'ingest', name: body.name})) or kb_dev ingest writes attacker-supplied bytes to an arbitrary path writable by the host process.

entry.js:60The plugin's real behaviour is executed from copies in the user-writable ~/.dsh/knowledge, refreshed only when the package file is newer. A hidden powershell process with execution-policy bypass runs it at activation when 'pinned' is set, or on demand through POST /knowledge/desktop, so the executed desktop code cannot be reviewed from this package.

impl.js:250engine.preview/ingestPath call fs.readBytes on an unvalidated absolute path (up to 256 MB) and return extracted text to the model, with no workspace restriction and 'danger-full-access' policies. A prompt injected through an ingested document could direct the agent to pull unrelated local files (configs, credentials) into the conversation sent to the model provider.
Local RAG plugin is reachable via 6 HTTP routes, model tools and a file-drop pump, and concatenates user-controlled file names into PowerShell commands, allowing command injection.
Rule (2026-10-28): when a dynamic conclusion exists it is authoritative (it may raise or lower the level); the static scan tier (vet primary, dsh backstop) applies only when there is none (not queued / audit failed / withdrawn). The audit never writes the exemption list (exemptions are manual only); a wrong conclusion is withdrawn by human review and the tier falls back to the static scan tier.
