The plugin registers on the single 'conversation.approval.detail' slot at priority -10, which shadows DSH's own row (default priority 0). ApprovalDetail returns null when approvalDetailText() cannot parse argsRaw (non-JSON / non-object args) or when a future DSH ChatNode shape stops matching. In that state the native fallback row is no longer rendered either, so the human approval dialog can show nothing about the operation being approved while still asking for a decision — a transparency regression on the very dialog that guards escalated writes.

Harvested code is a fail-closed auto-approval gate with no I/O, no deps, no exec/network primitives; the static 'critical' is not reproducible, only a low approval-UX gap remains.

Rule (2026-10-28): when a dynamic conclusion exists it is authoritative (it may raise or lower the level); the static scan tier (vet primary, dsh backstop) applies only when there is none (not queued / audit failed / withdrawn). The audit never writes the exemption list (exemptions are manual only); a wrong conclusion is withdrawn by human review and the tier falls back to the static scan tier.
