install.mjs:624install.mjs silently appends `mcp__dsh` to Claude Code's permissions.allow, so every dsh_task/dsh_task_cancel call runs without the per-call confirmation the user normally gets. Those calls start a `dsh --profile subagent` process with DEFAULT_PERMISSION 'danger-full-access' (lib/tasks.mjs:39) and profile approval policy 'never' (profile/cordis.patch.yml:36). An LLM steered by untrusted repo content (prompt injection) can therefore reach unconfirmed OS command execution outside the workspace; the removed confirmation is the only human checkpoint in that chain.

lib/monitor-host.mjs:344When there is no fresh observer heartbeat and no GUI host process (and the probe succeeds), ensureMonitorHost spawns a GUI application detached from the task. The launch is gated by DSH_SUBAGENT_AUTOSTART_MONITOR, cooldown and heartbeat/host checks, and never fails the task, but it is an unexpected process launch triggered by an MCP call. DSH_SUBAGENT_MONITOR_CMD/DSH_SUBAGENT_MONITOR_ARGS further let the environment redirect the spawned executable.
Installer silently allow-lists mcp__dsh while tasks run with danger-full-access and approval:never, so harness-driven commands execute unconfirmed; no backdoor or exfiltration found.
Rule (2026-10-28): when a dynamic conclusion exists it is authoritative (it may raise or lower the level); the static scan tier (vet primary, dsh backstop) applies only when there is none (not queued / audit failed / withdrawn). The audit never writes the exemption list (exemptions are manual only); a wrong conclusion is withdrawn by human review and the tier falls back to the static scan tier.
